Patient Consent Form

• An important element of the full EBCD process is the filming of short clips of patients and staff and family members talking about their experience of using a service.  It’s great to be able to do this when patients or staff or family members are happy for that to happen, but it is very important that we obtain written consent to do so. This protects you and them. It also allows us to potentially re-use the video for future publicity of our work. Without the patient’s written consent, we cannot agree to that.
