The 3 year Plan for the Stroke service

Our aim for the next 3 years will be to sustain and continue to build on improvements in management of hyperacute stroke while developing greater focus on increasing the reliability of inpatient care delivery for patients who are living with a stroke . This will including reliable prevention of further harm (IPC preventing DVT) or risk (identification and treatment of AF), maximising rehabilitation, MDT decision making and discharge planning ensuring optimal return to home in shortest necessary time. Days spent at home in first 90 days after stroke will be used as a key outcome measure. Key to identifying and driving improvement patient and family experience will be central to all of this activity. We plan to increase involvement of patients, families, community partners and the third sector in the next phase of improvement work.
